Adoption of Integrated Care Pathways (ICPs)

ICPs - also known as care profiles or protocols, determine locally agreed (multidisciplinary) healthcare practice, based on guidelines and evidence (where available) for a specific patient group. In Healthcare@Home, we have built a portal to support the various phases of the Wales Diabetes NSF. This portal is intended for use by clinicians enacting the care pathway and acts as a decision support tool aiding clinical staff to deliver the appropriate treatment / intervention to a patient given their current medical condition. Adoption of ICPs enables consistency in provision of medical care across multiple clinical sites.
